Bleeding after a bowel movement, itching, burning or pain around the anus can point to haemorrhoids, an anal fissure or another problem. The exact cause can only be determined by a proctological examination.

Explanation

What are haemorrhoids

Haemorrhoids are vascular cushions in the anal area. A problem arises when they enlarge and start to bleed, hurt or prolapse. We distinguish two basic types.

Internal haemorrhoids

They are located inside the rectum. They often do not hurt, but they can cause:

  • bright red blood after a bowel movement
  • pressure in the rectum
  • itching and moisture
  • prolapse during a bowel movement

It is precisely internal haemorrhoids that can often be treated on an outpatient basis without conventional surgery.

“External haemorrhoids”

They are located around the anal opening and may be visible or palpable. They often manifest as pain, swelling, itching or a tender lump.

Neither Barron ligation nor infrared coagulation is intended for this type of haemorrhoid. The doctor will recommend a suitable solution after the examination.

Extent of the finding

Grades of internal haemorrhoids

Grade I

The haemorrhoids remain inside but may bleed.

Grade II

They prolapse during a bowel movement and return on their own.

Grade III

After prolapsing, they can be pushed back in. And they remain inside the rectum. It is high time to see a doctor. And to set aside embarrassment, fear and hesitation.

Grade IV

They remain permanently prolapsed.

The earlier haemorrhoids are addressed, the greater the chance of using gentle outpatient treatment.

Another diagnosis

Anal fissure

An anal fissure is a small tear in the lining of the rectum. Typical symptoms include sharp pain during a bowel movement, burning and sometimes a small amount of bright red blood.

Fresh fissures can often be treated by adjusting the stool, with proper hygiene and topical preparations. For long-term problems, the doctor will recommend a further course of action.

Bleeding can be caused by a haemorrhoid or an anal fissure, but also by another condition. It is therefore not advisable to automatically assume that it is
only haemorrhoids.

Depending on what is needed, this is followed by a visual examination, a digital examination or an inspection of the anal canal using an anoscope.

How we treat

How we treat haemorrhoids

Barron ligation

Barron ligation, sometimes called rubber banding of haemorrhoids, is used to treat internal haemorrhoids. The doctor places a small elastic band over the haemorrhoidal node, which cuts off its blood supply. The tissue then shrinks and separates off naturally.

The patient may feel pressure, an urge to pass stool or temporary discomfort.

Infrared coagulation

During infrared coagulation, the doctor treats the haemorrhoidal tissue with targeted infrared light. This closes off the small blood vessels and gradually shrinks the haemorrhoid.

The method is used especially for smaller internal haemorrhoids and bleeding.

Combination of methods

In some patients, the doctor may recommend combining Barron ligation and infrared coagulation. It depends on the type, number and size of the haemorrhoids.
